The undersigned confirms and acknowledges that:

1. The terms of payment are Net 30, due within 30 days of the invoice date.

2. A service charge in the amount of 2% per month shall be rendered on any overdue balances.

3. The credit limit, payment terms, and overdue interest rate may change from time to time depending on circumstances. The applicant shall be notified of any changes in writing and shall be bound by these changes.

The undersigned further declares that the statements made herein are to the best of my/our knowledge true and complete. The undersigned authorizes FVSTAR or any credit bureau or other investigative agency employed by FVSTAR to investigate the references herein listed as well as any statements or other data obtained by any other person pertaining to the applicants credit and financial responsibility. The undersigned agrees to notify FVSTAR of any material change in the condition of the applicants affairs and this statement should be construed by FVSTAR to be a continuing statement of the condition of the undersigned until written notice to the contrary is received by FVSTAR.

The undersigned understands that they will be in default if they fail to pay for any sales and/or services when due, and in the event that it becomes necessary for FVSTAR to incur collection costs or to institute legal proceedings to collect any amount due under this agreement or any portion thereof, I/we promise to pay such additional collection costs, charges, and expenses including reasonable attorneys' or solicitors' fees if the account is placed in the hands of attorneys or solicitors for collection. We hereby authorize FVSTAR or its agent to register or record, and re-file, re-register or re-record any moneys in such offices as FVSTAR may determine and wherever required or permitted by law, for the protection of FVSTAR's money.
